Intentional Walk Gone Wrong
Last night the Orioles (my favorite team; yes, I love pain) blew a 5-1 lead in the ninth inning against the Marlins to send the game into free baseball. What O's reliever Todd Williams did in the 10th far surpassed the embarrassment of Chris Ray's first blown save of the season.
With the game-tying run on second, Williams attempted to intentionally walk Miguel Cabrera. Instead, he served up a softball style lob which Cabrera hit back up the middle to drive in the game-winning run. Seriously, I'm not making this up, it really happened. Watch for yourself.

Coach Sam Perlozzo said after the game, "Well, I don't know. He, ah, has a little trouble with his intentional walks. I don't know if it's a phobia or what it is. He left the ball out over the plate, I guess."
I have only seen someone hit an intentional walk pitch once in my life. I was 11 years-old and this 6'4" kid who claimed to be 12 hit one not just over the fence, but over the seven-story sound wall beyond the fence and onto the Beltway. It probably went about 350 feet.
