A six-year-old Hindu devotee, with his mouth pierced with a trident, takes part in an annual religious procession called Shitla Mata in the northern India city of Chandigarh, August 13, 2006. Hindu devotees subject themselves to painful rituals during the religious procession to demonstration their faith and penance to the deity at a temple dedicated to goddess Shitla.

    Not all Hindus do painful rituals like this.  In fact, the vast majority do not.  Rituals like this one probably take place in the few unmodernized villages left in India.
    Also, if you think about it, it's not all that different from a lip piercing that many people have.
